Comments:
An unannounced monitoring inspection was conducted with center staff on April 23, 2019 to determine compliance with the licensing standards. There were 17 children, ages four through twelve, present with two staff. The children were arriving from school, having afternoon snack, and playing outdoors. Snack served today: graham crackers and water. Please serve components from two of the four food groups each day. Two staff files and five children's files were reviewed - please see Supplemental Summary for missing items. The last health inspection was conducted 1/30/2018 - please call to schedule an inspection. The last fire inspection was conducted 3/12/2019. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review of two staff files, the facility failed to have documentation of a tuberculosis test or screening within 21 days of hire or 12 months prior to hire. Evidence: Staff A (date of hire 1/2/2019) did not have documentation of a tuberculosis test or screening on file.

Plan of Correction: Staff A was asked to complete the tuberculosis test or screening and is making an appointment.

Standard #: 22VAC40-185-260-B
Description: Based on review of administrative documents and staff interview, the facility did not have a current annual inspection from the health department. Evidence: the last documented health inspection was conducted 1/30/2018.

Plan of Correction: Center director contacted the health department to request a health inspection.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility failed to keep all hazardous substances such as cleaning materials in a locked place, preventing access by children. Evidence: a container of generic Clorox cleaning wipes was on the cabinet by the girls rest room, unlocked.

Plan of Correction: Center staff will keep hazardous substances locked.

Standard #: 22VAC40-185-560-F
Description: Based on observation and staff interview, the facility failed to provide food based on the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). The USDA guidelines require that components from two of the four food groups be served for snack each day. Evidence: the facility served graham crackers and water for afternoon snack.

Plan of Correction: Two components selected from the four food components will be served for afternoon snack.

Standard #: 22VAC40-191-60-B
Description: Based on review of two staff files, the facility failed to have documentation of a completed sworn disclosure statement prior to employment. Evidence: there was no documentation of a sworn disclosure statement for Staff C prior to hire (date of hire10/22/2018).

Plan of Correction: Staff C will complete a sworn disclosure statement.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of two staff files, the facility failed to have documentation of a completed search of the central registry (CPS check) within 30 days of hire. Evidence: there was no documentation of a search of the central registry check on file for Staff C (date of hire10/22/2018).

Plan of Correction: If the CPS check can not be located, it will be resubmitted.
