Comments:
An unannounced renewal inspection was conducted on 04/24/2019 from 4:00pm - 5:35pm. At the time of entrance there were 49 school age children in care with four staff. Children were observed eating dinner. The inspector additionally observed children playing table games in the cafeteria and outdoor games. Records were reviewed for eight children One violation was observed and is listed on the violation notice.

Standard #: 22VAC40-185-70-A
Description: Based on record review and interview, the center failed to ensure that staff provide the name, address and telephone number of a person to be notified in an emergency which shall be kept at the center. Evidence: 1. Emergency contact information was not available for staff 1. 2. Administrative staff confirmed that emergency contact information was not on file for staff 1.

Plan of Correction: Staff 1 will complete an emergency contact sheet today.
