Comments:
A subsidy health and safety inspection (SHSI) was conducted from 4:00 p.m. to 5:00 p.m. with the director and one staff. Two staff records were reviewed including staff training and CPR and first aid information. SHSI requirements were reviewed. Standard #: 22VAC40-665-520-B-11
Description: Based on review of children's records and the allergy list, the vendor failed to ensure there was a written allergy care plan for diagnosed food allergies from a physician in the record. Evidence: Child #5 has a diagnosed food allergy to 'peanuts, tree nuts, sesame seeds, raw egg' and there is no written allergy care plan from a physician that includes steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: This information is on file at the office and will be put in the child's file as soon as possible.
