Comments:
An unannounced monitoring inspection was completed on 04/30/2019. There were 19 children, ages 6 months-5 years, in the care of 5 staff members during the inspection. The inspector reviewed 5 children's records, 5 staff record and 1 medication during the inspection. The children were observed in the following activities: large motor play outdoors, during lunch and during rest time. Infants were observed being fed and allowed to sleep on demand. The inspector reviewed the emergency preparedness plans and the emergency supplies during the inspection. The inspector discussed the following with the staff: subsidy requirements, summer programming plans, staffing and toileting. The inspector arrived for the inspection at 9:20 a.m. and departed at 1:45 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review and discussion with the staff, the facility failed to obtain documentation that each child has received the immunization required by the State Board of Health before the child can attend the center. Evidence: 1. The record for Child #4 did not contain documentation of the child's immunizations required by the State Board of Health. The documented start date for Child #4 was 04/11/2019. Child #4 was present during the inspection.

Plan of Correction: The program director will text the parent today to have those faxed over.

Standard #: 22VAC40-185-330-B
Description: Based on observations and measurements, the facility failed to ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards under equipment with moving parts or climbing apparatus to create fall zone free of hazardous obstacles. Fall zone are defined as the area underneath and surrounding equipment that requires a resilient surface and should include the child's trajectory in the event of a fall while the equipment is in use. Evidence: 1. The fall zone for the red swing set does not contain the required resilient surfacing for the required fall zone. The swing set is 7 feet tall and requires a 14 foot fall zone in the front and back of the swing set. The area in behind the swing set (closest to the parking lot) only has the required depth of resilient surfacing for 9 foot of the required 14 foot fall zone and the area in front of the swing set (facing the other swing set) only has the required depth of resilient surfacing for 6 feet of the required fall zone. 2. The fall zone for the red swing set has a wooden picnic table within the required 14 foot fall zone; the table is 10 feet from the swing set. 3. The fall zone for the wooden swing set does not contain the required resilient surfacing for the required fall zone. The swing set is 7 feet tall and requires a 14 foot fall zone in the front and back of the swing set. The areas behind and in front the swing set only has the required resilient surfacing for 6 feet of required 14 foot fall zone.

Plan of Correction: The mulch will be spread out.

Standard #: 22VAC40-185-350-E-2
Description: Based on observations and record review, the facility failed to ensure that for children ages 16 months-2 years there is one staff member for every five children. Evidence: 1. 16 children were observed playing on the playground. There were 3 staff members supervising the 16 children. Child #2 was identified to be the youngest child on the playground. Record review determined that Child #2 was 16 months old. 2. 16 children were observed preparing to nap in the same classroom. There were 3 staff members supervising the 16 children. Child #2 was identified to be the youngest child. Record review determined that Child #2 was 16 months old. 3. The required staff:child ratio for children 16 months-2 years is 1 staff member: 5 children.

Plan of Correction: The program director will separate the children during outside playtime and an additional staff will come in before nap.

Standard #: 22VAC40-185-450-A
Description: Based on observations, the facility failed to ensure that during designated rest periods children shall have linens consisting of a top cover and bottom cover. Evidence: 2 children were observed with only one cover during the designated rest period.

Plan of Correction: The program director will make sure that every child has 2 covers.

Standard #: 22VAC40-185-540-E
Description: Based on observations and discussion with the staff, the facility failed to maintain the required nonmedical emergency supplies required by the standards. Evidence: There was not a working battery-operated radio on site during the inspection.

Plan of Correction: The program director will buy a new radio.

Standard #: 22VAC40-185-570-E
Description: Based on observations and discussion with the staff, the facility failed to ensure that prepared infant formula shall be refrigerated, dated and labeled with the child's name. Evidence: Child #1 was observed being bottle fed. The bottle was not dated nor labeled with the child's name.

Plan of Correction: The staff will label and date the bottles.
