The inspector reviewed standard 330-B with the director.

Comments:
The licensing inspector conducted an unannounced monitoring inspection on 04/29/2019 from 09:31am to 3:25pm. The children were observed playing on the playground, participating in story time, singing and eating morning snack. A formal observation was completed with the two and four year old classrooms. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with the children in each classroom when appropriate. All classrooms and playgrounds were inspected today as well as the mini buses. Ten children?s records and ten employee records were reviewed during this inspection. Medication is being administered and was reviewed. The center?s injury prevention plan was updated 03/2019. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 3/28/2019 Last shelter-in-place drill: 09/17/2018 Last fire inspection: 01/30/2019 Last health inspection: 2/25/2019 Today, the following child to staff ratios were observed: Infants - 3:1 Toddlers ? 12:3 Twos - 14:2 Twos - 8:1 Threes - 8:1 Threes - 5:1 Threes - 7:1 Threes and Fours - 10:1 Fours and Fives ? 10:2 Fours and Fives ? 9:1 Fours and Fives ? 9:1 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-240-C
Description: Based on review, the center did not ensure two of ten staff records contained documentation of the required 16 hours of annual training hours. Evidence: 1. The record of staff #4 (start date: 3/26/18) had documentation of 5 annual training hours. 2. The record of staff #8 (start date: 1/12/12) had documentation of 10 annual training hours.

Plan of Correction: Training hours will be reviewed this week and addressed as needed.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. The ballroom had 2 cleaners located in an unlocked bathroom out of the reach of children. 2. Room #7 had cleaners located in an unlocked closet out of the reach of children. 3. The staff bathroom had one cleaner located on a shelf out of the reach of children. The staff bathroom was unlocked. 4. One of the center's buses had an unlocked cleaner located behind the drivers seat in a large pocket within the reach of children.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-185-340-A
Description: Based on observation and an interview, the center did not ensure care, protection, and guidance when supervising children. Evidence: The inspector observed an infant sleeping in a bouncer seat. The bouncer seat was located in the infant's assigned crib. The teacher informed the inspector that the child fell asleep in the bouncer seat and the teacher picked the child up while asleep and placed the infant, while strapped in the infant seat, into the crib. The bouncer seat had the following manufacturer guidelines listed; Always use on the floor, Do not use for long periods of sleep, Never move while child is asleep.

Plan of Correction: The seat will be removed from the infant room. The directions will be reviewed before placing any equipment in the infant room.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center did not ensure that disposable diapers were disposed in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: The infant classroom's foot operated trash can was inoperable.

Plan of Correction: The trash can will be replaced this evening.
