Comments:
The licensing inspector conducted an unannounced monitoring inspection, at which play groups, lunch time, personal hygiene, rest time, and activities were observed. Children were observed; and staff and children files and other documentation were reviewed. Forty (40) children, aged 6 months-5 years, were found to be in care with 6 staff, kitchen staff, and administrative staff. Previous violations were reviewed, and had been corrected. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 12:30pm and concluded at 3pm.

Violations:
Standard #: 22VAC40-185-240-A
Description: Based on review of staff files the facility failed to maintain compliance with this standard as required relating to staff orientation. Evidence: Staff record #1, hire date 3/25/19, did not have documentation of orientation as required. Staff record #4, hire date 4/01/19, did not have documentation of orientation as required.

Plan of Correction: These forms will be obtained from HR and placed within the file for future review. All staff files will be reviewed for compliance, and updates made as needed, to ensure future compliance.

Standard #: 22VAC40-185-280-B
Description: Based on observations the facility failed to maintain compliance with this standard as required relating to locking of hazardous items. Evidence: The kitchen door was not locked, which allowed access to the back storage room which houses the maintenance storage area, which has numerous hazardous substances accessible to the children in care.

Plan of Correction: Maintenance will be contacted and asked to install a half door that is locked as to prevent access to the hazardous area. Until then the kitchen door will be closed, when no one is present in the kitchen, to help prevent any access.

Standard #: 22VAC40-185-330-A
Description: Based on review of the playground the facility failed to maintain compliance with this standard as required relating to the upkeep of the resilient surfacing. Evidence: The rubber mats installed as resilient surfacing under and around the big climber, on the playground used by the children, are starting to raise up on the corners which poses a tripping hazard. Also there is a space at the end of the slide where the 2 mats have separated and allows for an opening to the hard surface underneath. The opening allows for a childs foot to become wedged in and poses a hazard.

Plan of Correction: Maintenance will be contacted so that they can review the options of correcting the hazards, and act upon the option chosen. Staff will continue to review the playground for future hazards and act accordingly to rectify any noted.
