Technical Assistance:
Discussed updating emergency preparedness plan , soiled linen receptacle and update daily routine with specific rest time.

Comments:
Licensing inspector arrived at 12:15 pm and departed at 2:45 pm to conduct a licensing inspection and a Subsidy Health and Safety inspection. At the time of the tour there was fifty nine children in care with 7 staff . Children were observed eating lunch, nap time and participating in activity on the fun bus. The sample size was five staff records and six children records. As of May 1, 2019, corrections have not been received.

Description: Based on record review, the licensee did not ensure at least every two years from the date of the first initial screening or testing, to resubmit tuberculosis screening results. Evidence: During the inspection conducted on April 25, 2019, staff #1 most recent tuberculosis screening was 03/12/2017 which is more than two years old.

Description: Based on observation, the licensee did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: During the inspection conducted on April 25, 2019 , the licensing inspector observed the following inside and outside the building: - In the Pre-K classroom bathroom, the divider is rusting and there was red rust flaking on the floor. - In the Pre- K classroom , there was white peeling paint on the door frame. - Outside on the preschool playground area, the wooden door frame to enter preschool classroom and the frame at the bottom of the outside window of the preschool the bottom is deteriorating the wood is broken and there is wholes on the frames and peeling paint was observed on both sides of building in multiple areas of the building - On the preschool playground composite there was multiple areas of red and blue peeling paint.

Description: Based on observation, attendance record review and staff interviews , the licensee did not ensure that for infants, there was one staff member for every four children. Evidence: During an inspection on April 25, 2019 , the licensing inspectors observed seven children napping ages 15 months to 21 months at 12:40pm and 1 staff present. Attendance record was reviewed and confirmed with director the classroom was not meeting the required 1:4 ratio.
