Comments:
Licensing inspector arrived at 12:15 pm and departed at 2:45 pm to conduct a licensing inspection and a Subsidy Health and Safety inspection. At the time of the tour there was fifty nine children in care with 7 staff . Children were observed eating lunch, nap time and participating in activity on the fun bus. The sample size was five staff records and six children records. As of May 1, 2019 corrections have not been recieved.

Standard #: 22VAC40-665-540-A
Description: Based on record review, the licensee did not ensure prior to coming in contact with children and the documentation has been completed within the last 30 calendar days of the date of employment communicable tuberculosis (TB)is determined to be free of communicable tuberculosis (TB). Evidence: During the inspection conducted on April 25, 2019, staff #3 hire date March 25, 2019 tuberculosis screening results were completed 01/12/2019 which was not completed within the last 30 calendar days of the date of employment. Staff #2 hire date March 19, 2019 and tuberculosis test results were completed March 20, 2019 . The center did not have staff #2 tuberculosis screen before coming in contact with children.

Description: Based on staff interview, the vendor did not ensure there was a written policy and procedures that that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. Evidence: During the inspection conducted on April 25, 2019, staff #5 was not able to provide a written policy and procedures.
