Comments:
An unannounced monitoring inspection was conducted on April 29, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 10:15AM and concluded the inspection at 12:15PM. The center's director assisted the inspector throughout this inspection. There was a total of 66 children in attendance. During the inspection, children were observed singing, letter and journal writing, and cleaning up to from center time. Three groups of children were observed outside taking a wagon ride, playing on the climbing equipment and exploring bubbles with their teachers. The center had the following posted: license, daily schedules, emergency numbers, evacuation maps, and the menu. The center's first aid kit and non emergency supplies were inspected. Medication, authorizations and allergy action plans were inspected. Multiple staff have current CPR and first aid and medication administration training (MAT). Transportation is provided and the buses along with the transportation information were inspected. Seven staff records, six children's records, fire drill, shelter in place, and lockdown drills were reviewed during today's inspection. Violations were cited and are documented on the violation notice. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description: Based on review of children's records, the provider did not ensure each record contained documentation of immunizations prior to the child attending the child day center. Evidence: Child #2, began attending the center 4/26/19. Documentation of immunizations was not been received as of 04/29/2019.

Description: Based on a review of children's records, the center did not ensure that each child's record contains all required information Evidence: The record for child #1 did not contain documentation of annual parent confirmation that the required information in the child's record is up to date. Child #1 has a date of enrollment of 10/10/2016 and documentation of a last annual update of 08/23/2017.

Description: Based on observation, the facility did not ensure hazardous substances such as cleaning materials, were be kept in a locked place using a safe locking method that prevents access by children. Evidence: In the pre-k classroom, one spray bottle of sanitizing solution was in a closest that was not locked to prevent access by children.

Description: Based on observation, staff did not ensure children's hands were washed with soap and running water after toileting or diapering Evidence: After changing child A, staff placed child on the floor and did not wash the child's hands.
