Comments:
An unannounced monitoring inspection was conducted with center staff on April 29, 2019 to determine compliance with the licensing standards. There were 90 children present, ranging in age from 5 months to 12 years, with 8 staff supervising. The 3s group was outside playing on the playground. The 4s class was watching an age appropriate movie and the younger children were playing with age appropriate toys. Thirty three school age children arrived via school bus and center vans. Nine children's files and two new staff files were reviewed - please see Supplemental Summary for details. Snack served today: Ritz crackers or rice krispie treats. Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility failed to keep all hazardous substances such as cleaning materials in a locked place. Evidence: the bleach water solution was in an unlocked cabinet in the two year old room. The bleach water solution and Rustoleum spray paint were in an unlocked upper cabinet in the school age room.

Plan of Correction: Center staff will lock the cabinets with the hazardous substances.

Standard #: 22VAC40-185-350-E-4
Description: Based on observation, the facility failed to maintain the ratio of one staff for every ten children ages three years to five years. Evidence: upon arrival at 2:30 p.m. the licensing inspector observed 24 four year old children with two staff. At 3:45 p.m., there were 12 school age children present combined with 16 preschool children present with two staff supervising. At 4:00 p.m., there were 17 school age children and 16 preschool children present with two staff supervising.

Plan of Correction: Several staff called out sick today, so the staff were unable to maintain ratio.

Standard #: 22VAC40-185-560-F
Description: Based on observation, the center failed to follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA) for snack. Evidence: The facility only served one component from one food group for afternoon snack today. Two components are required.

Plan of Correction: Two components will be served.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of two staff files, the facility failed to have documentation of a completed search of the central registry within 30 days of employment. Evidence: Staff 1 (date of hire 1/21/2019) did not have documentation of a completed search of the central registry within 30 days of employment.

Plan of Correction: Center director contacted the VDSS Central Registry on April 30, 2019 to check on the CPS background check. The paperwork was returned to the facility, but the facility did not receive it. It will be resent.
