An unannounced monitoring inspection was conducted on April 30, 2019 from 11:05am until 12:15pm. Upon arrival to the facility, there were 41 children present with 11 staff members. The children were observed participating in age appropriate activities, hand washing ,Lunch and diapering. A sample of 5 children's records and 5 staff records were reviewed. The licensing inspector toured the facility and reviewed supervision, emergency supplies, practice drills, nutrition, physical plant, required postings, and administration. There were violations cited in the area of background checks and physical plant. An exit meeting was conducted with the facility director prior to the end of the inspection.

Standard #: 22VAC40-185-290-3 Description: Based upon observation and interview the provider did not ensure that the electrical outlets located inside of the facility had protective covers to prevent potential injuries of children. Evidence: 1. During the inspectors tour of the facility it was found that an electrical outlet located in the infant room of the facility did not have a protective cover. a. The electrical outlet without a protective cover could be a potential hazard to children. b. There were children in the infant room during the time the violation was observed. c. The director toured the infant room with the inspector and also identified and confirmed the violation. Plan of Correction: Will replace protective cover.
Standard #: 63.2(17)-1721.1-B-2 Description: Based on record review and interview, the center failed to obtain a fingerprint background check for any staff who will be alone with, in control of, or supervising children prior to employment. Evidence: 1. There was no fingerprint background check on file for Staff #1. a. Staff #1 has a hire date of 04/12/2019 and was observed caring for children in the licensed program during the inspection. 2. The program director confirmed that Staff #1 did not have a fingerprint background check Plan of Correction: Will complete fingerprints.
