Comments:
The licensing inspector conducted an unannounced renewal inspection on Tuesday, May 7, 2019 from approximately 10:35 AM until 3:00 PM. The following ratios were observed with a total of 129 children present during the inspection:
Young Infants - 4 children with 1 staff
Middle Age Infants - 7 children with 2 staff
Older Infants (Red Room) - 8 children with 2 staff
Transitional Toddlers (Red Room) - 10 children with 2 staff
Toddlers (Orange Room) - 15 children with 3 staff
Twos (Yellow Room) - 15 children with 2 staff
Younger Threes (Green Room) - 19 children with 2 staff
Older Threes (Blue Room) - 18 children with 2 staff
Fours (Purple Room) - 16 children with 2 staff
Fours & Fives (Gold Room) - 16 children with 2 staff
Get Well ? 1 child with 3 nurses

The children and staff were engaged in a variety of activities while the licensing inspector was on the premises. Children were observed playing outside, participating in music and movement, playing musical instruments, building with blocks, playing dress up, creating Mother's Day surprises, painting butterflies, participating in center time and playing with toys. Children were also observed having their diapers changed, taking bathroom breaks and washing their hands and eating lunch. Lunch consisted of grilled cheese sandwiches, tomato soup, baked apples and milk. Later children were observed resting at nap time. All areas of the center including classrooms, hallways, kitchen, bathrooms and outdoor play area were inspected. The center was equipped with toys and supplies and items were available to the children.

The following information was reviewed: injury reports, injury prevention plan, infant feeding schedules, daily attendance, infant daily sheets, practiced emergency drill logs, liability insurance policy, parent agreements, staff training information, required postings and menu. The center?s first aid kit and emergency supplies were inspected. Medication is administered, and medication and medication authorizations were reviewed. During the inspection, 10 children's and 10 staff records were reviewed.

Standard #: 22VAC40-185-40-G
Description: Based on review of 10 children's records and interview, the center did not ensure proof of one child's identity was viewed within the required time frame. Evidence: During the inspection, the licensing inspector observed the record for Child #6 (start date 10/1/18) did not contain documentation of viewing proof of a child's identity. Management stated during the inspection, they were aware that information was not obtained and the child was enrolled by management that is no longer with the center.

Plan of Correction: We had already identified this information was missing from the child's record. We have requested the information from the child's parent. Going forward, we will make sure all required paperwork is obtained and reviewed when a child enrolls at the center.
