The licensing inspector completed an inspection in response to a self-reported concern regarding forbidden action that occurred at the center on 04/01/2019. The Department was notified of the incident on 04/02/2019. The inspection was initiated on 04/08/2019 and completed on 05/07/2019. On 04/08/2019, the licensing inspector was on site from approximately 3:13 PM until 4:05 PM interviewing staff, conducting observations at the center and reviewing documentation. The preponderance of the evidence gathered during the investigation did not support violations of the standards. There are no violations resulting from this inspection.
