- Technical Assistance:
Technical assistance provided regarding physical plant, resilient surfacing, slides, swings, use zones.
An unannounced monitoring inspection was conducted beginning at 1:25 p.m. and ending at 5:15 p.m. with the director. There were 46 children ranging in age from 3 months to 5 years of age under the care and supervision of 7 staff. Infants were observed napping and in various play areas. Toddlers were observed waking from nap and having story time. Two's, three's, four's and five's were observed during rest time. The playgrounds were observed. Six children's records and four staff records were reviewed. Emergency drills were reviewed and up to date. There are no medications on site. There are three staff current in Medication Administration Training (MAT). Standard #: 22VAC40-185-160-A Description: Based on review of documentation, the center failed to have documentation of Tuberculosis (TB) screenings within 21 days of employment. Evidence: Staff #1, date of hire 10/3/2018, did not have documentation of TB screening in the file. Staff #4, date of hire 3/18/2019, did not have a record. Plan of Correction: Both staff will obtain TB tests/screenings as soon as possible.
Standard #: 22VAC40-185-160-C Description: Based on review of staff records, the center failed to ensure staff members had repeat Tuberculosis screenings every 2 years. Evidence: Staff #2's most recent TB test was dated 3/27/2017. Plan of Correction: Staff #2 has had an updated TB test done and it will be read tomorrow.
Standard #: 22VAC40-185-70-A Description: Based on review of staff records and interview with staff, the center failed to maintain a complete record for all staff. Evidence: Staff #4, date of hire 3/18/2019, did not have a record. Plan of Correction: The record will be recreated immediately.
Standard #: 22VAC40-185-440-E Description: Based on observation, not all occupied rest mats had at least 12 inches of space between them. Evidence: In the Two's class, two occupied rest mats were observed touching at the corner. A third rest occupied rest mat was approximately 6 inches from the second. Plan of Correction: Staff will be retrained regarding placement of rest mats.
Standard #: 22VAC40-185-450-A Description: Based on observation, not all rest mats had a bottom cover and top cover. Evidence: In the Two's class, there was one occupied rest mat that did not have a top cover. There were two occupied rest mats that did not have bottom covers. In the PreK class, there were three occupied rest mats without bottom covers. Plan of Correction: Staff will be retrained.
Standard #: 22VAC40-191-60-B Description: Based on review of documentation, the center failed to ensure all staff had a sworn statement for all staff by the first day of employment. Evidence: Staff #1, date of hire 10/3/2018, did not have a sworn statement in the record. Staff #4, date of hire 3/18/2019, did not have a record. Plan of Correction: Staff #1 will complete the sworn statement today. Staff #4 completed the sworn statement during the inspection. Staff files were misplaced recently due to renovations.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of staff records, the center failed to ensure that staff had a complete central registry background check within 30 days of employment. Evidence: Staff #1, date of hire 10/3/2018, did not have documentation of a satisfactory central registry background check. Staff #4, date of hire 3/18/2019, did not have a record. Plan of Correction: Central registry background checks will be requested as soon as possible.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review of staff records, the center failed to request and out of state central registry background check for staff that resided outside of Virginia within the last 5 years. Evidence: Staff #1, date of hire 10/3/2018, indicated residence in California in the last five years and there was no documentation of a request for a California central registry background check. Plan of Correction: An out of state central registry background check will be requested.
