A unannounced monitoring inspection was conducted with the director and staff. 17 children were in care with a age range of two years to nine years old. Children were observed during free play, group games. Five staff files and six children's files were reviewed. All required paper work was posted. Three staff are current in MAT. Standard #: 22VAC40-185-70-A Description: Based on review of five staff records not all were complete. Evidence: Staff A 's T.B. test expired 5/6/19. Plan of Correction: We will have the staff take a T.B. test.
Standard #: 22VAC40-185-180-D Description: Based on review and staff interviews not all staff that drive the center van has disclosed any traffic violations that occurred five years prior or during employment as a driver. Evidence: Staff B has no documentation of a driving statement. Plan of Correction: We will has the staff write a statement.
Standard #: 22VAC40-185-320-B Description: Based on observation not all bathrooms used by children were equipped with paper towels. Evidence: The two bathrooms in the downstairs rooms did not have any paper towels. Plan of Correction: We will restock all bathrooms.
