Comments:
An unannounced Monitoring inspection was conducted in the center on March 5, 2019. As reported, 130 children are currently enrolled in the center. Upon the Licensing Inspector's arrival in the center at 11:00AM, there were 54 children present in the pre-K building, the children were grouped in 6 classrooms. The LI observed several groups of children during outdoor play on the fenced playground. Several groups of children participated in a finger-painting activity. The lunch meal was also observed. In the afternoon, 58 additional children arrived from school and were observed in building 2; 3 staff members were present with the 2 groups. The children were served a snack. A sample of 10 children's records and 4 staff records was reviewed. Documentation of the annual fire and health inspections was current. Medications were observed in the center. Documentation of the practiced emergency drills was reviewed. Findings of the inspection were reviewed at exit. The on-site inspection was conducted in the center from 11:00AM to 4:00PM. Violations:
Standard #: 22VAC40-185-140-B
Description: Based on documentation reviewed in one of ten records, the center accepted documentation of a physical examination which was completed prior to the child's attendance, the center failed to ensure the PE was within the prescribed time period: within 12 months prior to attendance for children two years of age through five years of age. Evidence: Documentation of the PE for child #9 was not within the prescribed time period. The child was 5 years of age at the time of .enrollment in 2019; the child's PE was dated 3/18/15.

Plan of Correction: The parent will be notified and requested to provide documentation of an up-to-date physical examination for the child.

Standard #: 22VAC40-185-150-B
Description: Based on documentation reviewed in a sample of 10 children's records, the center failed to ensure each physical examination and immunization record were signed by a physician, his designee, or an official of a local health department. Evidence: The PE and immunization record for child #9 were not signed by a medical professional or an official of a local health department.

Plan of Correction: The parent will be requested to obtain the appropriate signatures on the forms and return the signed copies for retention in the child's record.

Standard #: 22VAC40-185-60-A
Description: Based on a sample review of children's records, the center failed to ensure the record of each enrolled child contained the following information: 3.) when applicable, work phone number and place of employment of each parent who has custody. Evidence: The work phone number and place of employment of each custodial parent was not complete in three of ten (10) reviewed children's records. Information was not complete for children 1, 6, and 7.

Plan of Correction: The current information will be completed in each child's record.

Standard #: 22VAC40-185-320-B
Description: Based on inspection of the center's rest rooms, sinks in two of the rest rooms located in the pre-K building were not supplied with running warm water; that does not exceed 120 degrees F.

Plan of Correction: Maintenance will be scheduled to ensure warm water is supplied to the hand washing sinks.

Standard #: 22VAC40-185-350-A
Description: The center failed to ensure staff assigned to a group of children and counted in ratio was directly supervising the group. Evidence: Upon arrival in the center, the staff was out of the classroom and was in the front office area speaking with a visitor in the center. Staff reported that she was putting cots together that had been ordered for the center. The LI observed 15 children and one staff in the PreK classroom. The ratio is 1 adult to 10 children.

Plan of Correction: Staff will remain in the classroom supervising the group of children whenever they are counted in ratio.

Standard #: 22VAC40-185-350-C
Description: Based on direct observations, the center failed to ensure ratios were met when children are regularly in on-going mixed age groups, the staff to children ratio applicable to the youngest child in the group shall apply for the entire group. Evidence: 1. The LI observed two groups of school-age children, each group consisted of mixed ages. 2. For children, from age of eligibility to attend public school through 8 years, one staff for every 18 children (5.) The LI observed two staff members with a group of 38 children.

Plan of Correction: Teachers' and subs are currently being interviewed and hired. Will adjust staff scheduling to ensure classrooms maintain ratio until new staff are hired.
