Comments:
5/10/19 Arrived 9:50am Departed 10:40am An unannounced monitoring inspection was conducted today. Files were reviewed for five children and two staff. Fifteen children were present with two classroom staff. The children were planting flowers and working in centers. Thank you for your cooperation. Standard #: 22VAC40-185-270-A
Description: Based on a tour of the playground equipment, the facility failed to insure that all areas are in safe and operable condition. Evidence: 1. On the plastic climber where the green and yellow plastic panels meet to form the walls of the climber, the PVC tube used to hold together the panels is missing.

Plan of Correction: The center director stated that the pipe has been replaced but that the older children who play on the playground take the pipe out. The maintenance person was contacted to replace the pipe. The children will not play on the climber until the pipe is replaced.
