Comments:
An unannounced monitoring inspection was conducted on 05/07/2019 from 4:00pm - 5:35pm. At the time of entrance there were 26 school age children in care with two staff. Children were observed during afternoon program time which included dinner. Records reviewed for eight children in care. The inspector additionally reviewed medication and emergency supplies. The violation is listed on the violation notice and was reviewed with the program director at the conclusion of the inspection.

Description: Based on record review and interview, the center failed to ensure that the parent's authorization for medication, provided that it shall expire or be renewed after 10 work days. Long-term prescription drug use and over-the-counter medication may be allowed with written authorization from the child's physician and parent. Evidence: 1. The written authorization for one prescription medication and one over-the-counter medication expired on 04/16/2019. a. The parent and physician of child 1 had signed the medication authorization for these medications on 04/16/2018. 2. Administrative staff confirmed that an updated authorization form had not been obtained from the parent of child 1.

Plan of Correction: The parent of child 1 was asked to update the medication authorization at pick-up on the date of the inspection.
