Comments:
An unannounced mandated Renewal inspection was conducted today from 4:00pm to 5:15pm. There were 28 children present during this inspection and 4 caregivers. Children were observed tracing with an overhead projector, playing board games, reading, making artisan pop-up cards, and playing basketball and tag in the gym. Reviewed 3 staff files, 2 children's files, medications and authorizations, emergency evacuation drill log, first aid kit, attendance, and physical plant. Violations:
Standard #: 22VAC40-185-340-A
Description: Based on observation and interview with staff, staff who were supervising children, did not ensure their care, protection, and guidance . Evidence: Staff#1 was not aware of where a child in their care was or what they are doing. When a parent arrived to pick up their child, Staff#1 was asked where Child#1 was and their response was "I don't know". Child#1 was located in the gym with another staff member.

Plan of Correction: We will review and re-train staff on the passport tracking system
