Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on May 15, 2019 from 11:00 a.m. through 2:20 p.m. There were sixty-one children in the care of eleven staff in six classrooms. The medication and medication paperwork was reviewed for one child. Seven children's and eleven staff records were reviewed and updated. Areas of non-compliance may be found on the violation notice. Areas observed were: story-time, outside play, hand washing/bathroom and diapering procedures, educational choice activities, outside play, lunch and nap. Interactions between the children and staff were observed as appropriate and the children seemed comfortable with the daily routine and schedule. Standard #: 22VAC40-185-330-B Description: Based on observation and measurement the center failed to obtain six inches of resilient surfacing under the small slide on the outside playground. Evidence: 1. The mulch under the small slide measured less than three inches. 2. Staff confirmed the mulch was not at least six inches in depth under the small slide. Plan of Correction: Additional mulch will be ordered and places under the small slide to maintain at least a six inch depth.
