Comments:
An unannounced monitoring inspection was conducted on 5/16/19 from 9:55am until 12:35pm. At the time of entrance, there were 99 children in care with 17 staff present. A sample of 10 children's records and 10 staff records were reviewed. Children were observed participating in morning learning activities, playing outside, eating lunch and resting quietly during nap time. Infant feeding, toileting and hand washing procedures were also observed. Medication, first aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Three violations in the area of administration are identified on the violation notice and were discussed with the center director during the exit interview.

Standard #: 22VAC40-185-130-B
Description: Based on record review and interview, the center did not ensure that documentation of additional immunizations are obtained once every six months for children under the age of two years. Evidence: 1. The most recent immunization record for child 2, age 1 year, is dated 8/29/18. Child 1 has an enrollment date of 4/2/18 and an updated immunization record was due in February 2019. 2. The center director confirmed that updated immunization records for child 1 are overdue.

Plan of Correction: The center responded with the following: Updated immunization records for child 2 will be requested from the parent. Going forward, the center will ensure updated immunization records are obtained every six months for children under age two.

Standard #: 22VAC40-185-140-A
Description: Based on record review and interview, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: 1. The record for child 1 does not contain documentation of a physical examination. Child 1 has a date of enrollment of 3/28/17 and was in care on the day of the inspection. 2. The center director confirmed that the record for child 1 was lacking documentation of a physical examination.

Plan of Correction: The center responded with the following: The parent was notified during the inspection that documentation of a physical examination was required.

Description: Based on record review and interview, the center did not ensure that each child's record contains all the required information. Evidence: 1. The record for child 1 does not contain documentation that the center viewed proof of the child's identity and age. 2. The center director confirmed that the record for child 1 did not contain proof of the child's identity and age.

Plan of Correction: The center responded with the following: The center has notified the parent of child 1 that proof of identity and age needs to be brought in to the center.
