Comments:
An unannounced monitoring inspection was completed on 05/17/2019 from 9:40 A.M.-1:00 P.M. At the time of the inspection 34 children were in care with ten staff and one administrator present. Five children records, eleven staff records, three medications and required documentation, emergency evacuations, first aid kits, emergency supplies, playground, and posted required information were reviewed. Children were observed during one-on-one interactions with infants, diapering, handwashing, storytime, art activity, outdoor playtime, snack time, handwashing, small group activities, lunch time, and departure. Description: Based on observation, the center failed to ensure that all electrical outlets were covered with protective . Evidence: 1. In the Two Year Old Bathroom there was one electrical outlet in the bathroom without a protective cover. 2. In the Three Year Old Classroom there were two electrical outlets without protective covers in the bathroom, and one electrical outlet without a protective cover in the classroom. 3. Staff verified that the electrical outlets were missing protective covers.

Plan of Correction: The staff immediately covered all electrical outlets with protective covers.

Standard #: 22VAC40-185-510-E
Description: Based on a review of medication, the center failed to ensure that all medication was labeled with the dosage amount, and the time to be given. Evidence: 1. Child 1M had a bottle of Benadryl that was not labeled with the dosage amount and the time to be given. 2. Staff verified that the medication was not labeled with the dosage amount or time to be given.

Plan of Correction: Staff immediately labeled the medication with the required information. The administrator will review all required components to be completed when staff are accepting medication from the parent.
