Comments:
An announced intial inspection was conducted on 05/20/2019 from 9:30 A.M.-12:30 P.M. The facility was toured and the outdoor play area and classrooms were measured for capacity. Some items observed/reviewed during the inspection included: required posted information, parent pick-up procedures, emergency maps and procedures, first -aid kit, emergency supplies, three staff records, and emergency plan. A recommendation will be made for an initial six-month conditional Child Day Center license with a capacity of 32 children and an age range of two years through twelve years. The exceptions to compliance are noted on the violation notice. If you have any questions or concerns contact the licensing inspector at (540)-430-9257 for further assistance.

Description: Based on review of non-medical emergencies supplies, the center failed to have all required non-medical emergency supplies. Evidence: 1. The center did not have a flashlight on site. 2. Staff verified that there was not a flashlight on site.

Plan of Correction: Administration will purchase flashlights to place in all classrooms and in the office area.

Standard #: 22VAC40-185-550-C
Description: Based on observation, the center failed to ensure that emergency maps contained shelter-in-place procedures/maps. Evidence: 1. None of the emergency maps in the center contained the shelter-in-place location. 2. Staff verified that the emergency maps did not indicate the shelter-in-place location.

Plan of Correction: Administration will add the shelter-in-place location to all emergency maps.
