Comments:
An unannounced renewal inspection was completed on 5/20/19. There were 55 children present in four groups with ten supervisory staff present and two administrative staff present in addition to the staff in the classrooms. Five children's records and five staff records were reviewed. Nine medications were reviewed that were for six children in care. The inspector arrived at the center at 8:45 am and departed at 1:45 pm. The children were observed during arrivals, during morning snack, during outside times, during "work"time in the classrooms, during free time in the classrooms, during story times, during lunch, and during nap.

Violations:
Standard #: 22VAC40-185-60-A
Description: REPEAT VIOLATION Based on record review, the center failed to ensure that children's records contained documentation of all required information. Evidence: 1. The records of children #1 and #2 were missing the work phone numbers for the mother and the father of the child. There were work places documented for all four parents. The cell phone numbers of the parents were documented. 2. The record of child #5 was missing the work phone number for the father of the child. There was a work place documented for the father and a cell phone number for the father documented.

Plan of Correction: The director will have the parents document their work phone numbers.

Standard #: 22VAC40-185-240-D-1
Description: Based on observation, record review, and interview, the center failed to ensure that whenever the center has agreed to administer prescribed medications the administration shall be performed by a staff member or independent contractor who has satisfactorily completed a training program for this purpose approved by the Board of Nursing and taught by a registered nurse, licensed practical nurse, doctor of medicine or osteopathic medicine, or pharmacist; or administration shall be performed by a staff member or independent contractor who is licensed by the Commonwealth of Virginia to administer medications. Evidence: 1. There was a medication observed for a child in care which was an emergency rectal medication. There was no Medication Administration Training (MAT) certificate observed for any staff who had completed the training module to administer rectal medications. The director and assistant director were asked if any staff had completed this training to administer the medication and both stated that noone had completed the training.

Plan of Correction: The staff will complete the training module.

Standard #: 22VAC40-185-510-E
Description: Based on observation, the center failed to ensure that medications were labeled with the dosage amount and time or times to be given. Evidence: 1. There were three over-the-counter medications, all as needed and not administered to a child, that were not labeled with the dosage amount and the time or times to be given.

Plan of Correction: Each medication will be labeled with the required information.

Standard #: 22VAC40-185-550-D
Description: Based on record review and interview, the center failed to ensure that two shelter-in-place (tornado) drills were practiced each year. Evidence: 1. There was one shelter-in-place (tornado) drill documented for 2018. 2. The director verified that two drills were not completed for 2018.

Plan of Correction: The director will make sure that two drills are completed and documented for 2019.
