Comments:
An unannounced monitoring inspection was completed on 5/21/19. There were 131 children present. Ten children's records and ten staff records were reviewed. Three "emergency as needed" medications were reviewed. Two inspectors arrived at the center at 8:35 am and departed at 3:15 pm. The children were observed during breakfast, during free time in the classrooms, during outside time, during play time in the indoor play land, and during nap.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. There were cleaning supplies unlocked in the two year old classroom on top of the paper towel dispenser that was approximately five feet high. The cleaning supplies were: aerosol glass cleaner, aerosol hospital disinfectant spray, soap and water solution, and brand hard surface sanitizer spray. 2. There were two aerosol cans of cleaning agent observed on a shelf in the nap room on a shelf approximately four feet high. Children were occupying the room at the time the cleaners were observed.

Plan of Correction: The director will remind all staff to keep all cleaning supplies locked and check the rooms when staff enter the rooms.

Standard #: 22VAC40-185-440-B
Description: REPEAT VIOLATION Based on observation, the center failed to ensure that cots were identified for use by a specific child. Evidence: 1. There were two children occupying a cot that was not assigned to the correct child with the numbering system that was being used. 2. There were two cots observed in use by two different children and there was no way to identify the specific child using it.

Plan of Correction: This was corrected during the inspection. The staff labeled the cots that were not labeled and changed the numbers to correspond with the child using it for the other two.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center failed to ensure that diapering procedures were followed as required. Evidence: 1. Three diaper changes were observed. The staff person was observed spraying the sanitizing solution on a paper towel each time and wiping the diaper pad with that paper towel. The diaper surface was observed to be cleaned with soap and water after each change. The diapering surface is required to be cleaned and sanitized by spraying or dipping the surface and allowing it to dry according to the manufacturer's recommendations for sanitizing.

Plan of Correction: The assistant director retrained staff on the correct diapering procedures to spray it on the surface instead of the paper towel.

Standard #: 22VAC40-185-560-F
Description: Based on observation, the center failed to ensure that the menu was changed to list substituted foods. Evidence: 1. The menu for breakfast listed pears, whole wheat bagels, and milk. The children were observed to be served mandarin oranges, french toast bites, and milk. The menu was not changed at the end of the inspection to show the substitution for the fruit.

Plan of Correction: The director will talk with kitchen staff about posting substitutions.
