An unannounced monitoring inspection was conducted on 5/17/19 from 4:00pm - 5:00pm. At the time of entrance there were 40 school age children in care with eight staff. Children were observed participating in various activities in the classrooms, playing outside, doing homework, and eating snack. Records were reviewed for five children and for five staff during the inspection. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.
Standard #: 22VAC40-185-160-A Description: Based on a review of five staff records, it was determined that the facility did not ensure that each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment and shall have been completed within 12 months prior to or 21 days after employing. Evidence: 1. The record for staff #1(date of hire 2/25/19) did not contain documentation of a negative tuberculosis screening. 2. The record for staff #4(date of hire 11/19/18) did not contain documentation of a negative tuberculosis screening. 3. Staff #6 reviewed the record for staff #1 and staff #4 and confirmed that the documentation of a negative tuberculosis screening had not been received for either staff. Plan of Correction: The facility responded: Both staff will be sent to complete a TB screening. All new employees will complete a TB screening within 21 days of employment.
Standard #: 22VAC40-185-550-D Description: Based on a review of the emergency drill log and interview, it was determined that the facility did not ensure that a monthly practice evacuation drill is completed. Evidence: 1. The emergency drill log did not have written documentation to demonstrate that an emergency evacuation drill was completed during the month of April 2019. The last entry for a completed emergency evacuation drill was for March 2019. 2. Staff #6 (Program Director) confirmed that there was no emergency evacuation drill completed during the month of April 2019. Plan of Correction: The facility responded: Going forward, we will ensure that a practice evacuation drill is completed each month and recorded on the emergency drill log.
