Comments:
An unannounced monitoring inspection was conducted today from 11:45am-2:15pm. A total of 49 children were present with 8 staff. Children were observed during various indoor activities, and an outdoor picnic. Transitions to naptime were observed with the toddlers. A sample of 5 children's records, 5 staff records, emergency supplies, and emergency drills were reviewed. Areas of non-compliance are documented on the violation notice. Standard #: 22VAC40-185-60-A
Description: Based on a sample review of records, 2 out of 5 children's records did not include the required information. Evidence: 1. The record for Child #2 was missing the name, address and phone numbers of 2 emergency contacts and documentation of verifying the child's identity and age. 2. The record for Child #3 was missing the name, address and phone numbers of 2 emergency contacts.

Standard #: 22VAC40-185-70-A
Description: Based on a sample review of records, the record for 1 out of 5 staff did not include the required information. Evidence: The record for Staff #5, who was identified as a program lead, did not include written information to demonstrate that the individual possesses the education or certification required by the job position.

Plan of Correction: A request was submitted on 5/7/19 Will forward as soon as copy is received

Standard #: 22VAC40-185-290-3
Description: Based on observations, unused electrical outlets did not have protective covers installed. Evidence: 5 unused outlets on a surge protector and 2 unused wall outlets at the entrance of the program did not include protective covers. 4 unused outlets on a surge protector in the lunch/snack room also did not have protective covers installed.

Plan of Correction: All outlets were checked and installed

Standard #: 22VAC40-185-560-G
Description: Based on observations, food brought from home was not properly labeled. Evidence: 9 out of 15 lunches were not labeled with dates and/or children's names in a toddler classroom. 4 out of 5 lunches were not labeled with dates and/or children's names in a preschool classroom.

Plan of Correction: Will continue to monitor this at school; an email was sent to the parents as well

Standard #: 22VAC40-191-60-C-2
Description: Based on a sample review of records, 1 out of 5 staff records did not include central registry findings within 30 days of employment. Evidence: There were no central registry results available for Staff #1 [date of employment: 9/17/2018] during the inspection.

Plan of Correction: Made a phone call to the registry 5/9/19, waiting for answer.
