Comments:
An announced monitoring inspection was completed on May 21, 2019 from approximately 12:30p.m. to 2:30p.m. Fifty children were in care and five staff were on premises during the inspection. Staff and children were observed in classrooms engaged in activities to include nap time, hand washing, toileting and arrival from school. Interviews and interactions were conducted and five children?s records and five staff records were reviewed. The Owner/Director was available for the inspection and present at the exit interview at which time inspection findings were reviewed. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation of the outdoor space, the licensee failed to ensure that areas and equipment of the center are maintained in a clean, safe and operable condition. Evidence: 1. One side of the building was observed creating a wall for the playground, which is accessible to children. Two pieces of metal siding on the wall were loose, creating sharp, hazardous edges. 2. Administrator #1 acknowledged this hazard.

Plan of Correction: The Owner will have someone repair the siding.

Standard #: 22VAC40-185-330-B
Description: Based on observation of the playground, the licensee failed to maintain resilient surfacing under equipment with moving parts or climbing apparatuses. Evidence: 1. A multicolored piece of metal climbing equipment, located on the playground, approximately six feet tall at the highest point and surrounded by mulch, lacked an adequate depth of mulch under moving parts and climbing apparatuses. 2. A multicolored piece of plastic climbing equipment, located on the playground, approximately one foot tall at the highest point and surrounded by mulch, lacked an adequate depth of mulch under moving parts and climbing apparatuses. 3. A metal see-saw with moving parts, surrounded by mulch, lacked an adequate depth of mulch under moving parts and climbing apparatuses. 4. Each of these pieces of equipment required at least six inches of mulch, but measurements taken ranged from zero to two inches of mulch. 5. Administrator #1 acknowledged the mulch was not the required depth.

Plan of Correction: The Owner will order mulch from Yardworks. In addition, over the summer the Owner will look into a long term fix such as building a retaining wall around the equipment that require resilient surfacing.

Standard #: 22VAC40-185-550-M
Description: Based on a review of records, the licensee failed to maintain all required elements of a written record of children's serious and minor injuries in which entries are made the day of occurrence. Evidence: 1. All injury reports since the beginning of the year were reviewed. 2. The injury reports lacked the date, time and means as to how the parents were notified of the injury and any future action. 3. Administrator #1 acknowledged these reports lacked this information.

Plan of Correction: The Owner will add the missing elements to the injury report form.
