Licensing Inspector arrived at 12:15 and departed at 1:45 PM to conduct an unannounced monitoring inspection in conjunction with a subsidy health and safety inspection. At the time of the inspection, there were 25 children (ages infant through four years) in care with four staff. All children were sleeping during the inspection. There is currently no medication being stored or administered on site. The sample size was nine staff and six children's records.

Standard #: 22VAC40-185-130-A Description: Based on review of six children's records, it was determined that the facility did not ensure that all children had documentation of required immunizations prior to attending the facility. Evidence: The following children did not have documentation of immunizations on file: - child #4 (date of enrollment 1/25/19) - child #5 (date of enrollment 3/4/19) - child #6 (date of enrollment 3/4/19) Plan of Correction: Center management will obtain immunization records for these children and ensure that no children begin care with immunization records on file.
Standard #: 22VAC40-185-140-A Description: Based on review of six children's records, it was determined that the facility did not ensure that all children had documentation of a completed physical on file within 30 days of attending the facility. Evidence: The following children did not have documentation of physicals on file: - child #4 (date of enrollment 1/25/19) - child #5 (date of enrollment 3/4/19) - child #6 (date of enrollment 3/4/19) Plan of Correction: Center management will obtain physicals for these children and ensure that no children have physical records on file within 30 days of enrollment.
Standard #: 22VAC40-185-60-A Description: Based on a review of six children's records, it was determined that the facility did not ensure that children had all required elements in their records. Evidence: The records for child #1 (date of enrollment 4/3/19) and #2 (date of enrollment 4/23/18) included only one emergency contact person rather than the required two. Plan of Correction: Center management will continue to review records for completion and add any missing information.
