Comments:
Health and Safety inspection was conducted to review supplemental health and safety requirements. There were 24 children in care with five staff. Children present ranged in age from 2years to five years old. Children were observed during nap time. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.

Standard #: 22VAC40-665-540-A
Description: Based on record review and interview the vendor failed to ensure that staff are evaluated by a health professional and are issued a statement that the indivisual is determined to be free of communicable tuberculosis. Documentation will be submitted at the time of employment and prior to coming into contact with children. The documentation will have been completed within the last 30 calendar days of the date of employment. Evidence: 1 - The record for staff 2 contains a tuberculosis evaluation that is dated 2/22/19. The documented date of employment for staff 2 is May 13, 2019. Therefore, the tuberculosis evaluation was outdated at the point of employment. 2 - Staff 1 stated that she thought that the tuberculosis test could be 12 months old at the point of employment.

Plan of Correction: Going forward the vendor will ensure that tuberculosis evaluations are completed within required time frames.
