Comments:
An unannounced monitoring inspection was completed on 5/23/19. There were 19 children present in three groupings with four staff at arrival and additional staff person who arrived in the late morning. Five children's records and five staff records were reviewed. The center does not administer medications. The inspector arrived at the center at 9:00 am and departed at 1:00 pm. The children were observed during structured learning activities, during free time in the classrooms, during outside time on the playground, during water play activities outside (all three groups of children) and during a picnic lunch (all three groups of children). There was discussion with the assistant director about the following: wading requirements (written permission, posted safety rules and emergency procedures, wading pools not allowed for children not potty trained), detailed discussion about requirements for the program director to be on site 50% of the operating hours, requirements for application of and authorization for topical ointments, a May fire drill will need to be completed and documented, a second shelter-in-place will need to be completed and documented for 2019.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to ensure that all required information was documented in each child's record. Evidence: 1. The records for children #2 and #4 did not have a first date of attendance documented. 2. The record for child #5 was missing a complete address for the second required emergency contact. 3. All five children's records were missing the required written agreement with parents that in addition to reporting communicable diseases in the household within 24 hours, life threatening diseases must be reported immediately.

Plan of Correction: The assistant director will document the child's first date of attendance and have the parent document the complete address. The director will add the agreement to the enrollment form for the next program year.

Standard #: 22VAC40-185-70-A
Description: Based on record review, the center failed to ensure that all required information was in each staff person's record. Evidence: 1. The record of staff #4 was missing a hire date and written information to demonstrate that the individual had the required orientation training.

Plan of Correction: The director will have all required information documented.

Standard #: 22VAC40-185-260-A
Description: Based on record review, the center failed to ensure that there was an annual fire inspection report from the appropriate fir official having jurisdiction. Evidence: 1. The last documentation of a fire inspection of the facility was 2/12/18.

Plan of Correction: The director will contact the public safety department to get the latest fire inspection report.

Standard #: 22VAC40-185-290-2
Description: Based on observation, the center failed to ensure that fans that were used were out of the reach of the children. Evidence: 1. There was a box fan in use during the inspection located at the entrance door to the facility. Seven toddler, two year old, and three year old children were observed going in and out of this door several times during the inspection. The fan was sitting on the floor and the children were observed walking directly by, within a few inches, of the fan on each occasion.

Plan of Correction: The assistant director will not use the fan unless it is out of the reach of the children and the cord is secured to not create a tripping hazard.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center failed to ensure that a diaper changing table or counter top designated for changing was used for children under three years of age. Evidence: 1. Two children under the age of three were observed changed standing up during two different rounds of diaper changes in the morning and in the afternoon. The diaper changing table or a counter top was not used to change the children.

Plan of Correction: The diaper changing pad will be used for all children under three in the future.

Standard #: 22VAC40-185-510-J
Description: Based on observation, the center failed to ensure that all medication was locked. Evidence: 1. There was a bottle of ipubrofen that was observed unlocked on a shelf in the entry way room. The medication was identified to be staff medication and was out of the reach of the children. The shelf was approximately five feet high.

Plan of Correction: This was corrected during the inspection. The ibuprofen was locked by a staff member.

Standard #: 22VAC40-185-550-C
Description: REPEAT VIOLATION Based on observation and interview, the center failed to ensure that shelter-in-place procedures were posted on each floor of the building. Evidence: 1. There were no shelter-in-place procedures on either of the two floors where children were in care.

Plan of Correction: The director will post these procedures.
