Comments:
An unannounced Monitoring inspection was conducted in the Center on May 20, 2019. The Licensing Inspector arrived in the center at 11:30AM. As reported 18 children are currently enrolled in the program. There were 14 children and 2 staff present at the time of the inspection. The children were observed in their classroom and in the school cafeteria during their lunch time. Staff were observed interacting with the children. Observed activities included: individual-selected center play, hand-washing procedures, lunch, nap time and dismissal. Staff also read a book to the children in circle time. A sample of 5 children's records was reviewed which included one record as a follow-up from the previous inspection. Documentation of fire and health inspections was reviewed. Staff had contacted the fire official for documentation of the current inspection. Findings were reviewed at exit. The inspection concluded at approximately 2:45PM. Standard #: 22VAC40-185-130-A
Description: Based on a review of children's records, the center failed to obtain documentation that each child had received the immunizations required by the State Board of health before attending the center. Evidence: Documentation of a current immunization record was not available for review in one of five records. Child #2 was 4 years old at the time of attendance, the immunization record noted the last immunization was given on 09/24/15 at the age of 10 months. The immunization record was not signed by a physician or his designee.

Plan of Correction: Staff will notify the child's parent and will request an updated immunization record. All records will be reviewed to ensure information is up to date in each child's record.

Standard #: 22VAC40-185-140-A
Description: Based on a sample review of children's records, the Center failed to ensure that documentation of a physical examination was obtained prior to the child's attendance or within one month after attendance. Evidence: 1. One of four children's records reviewed did not have documentation of a physical exam, child #2's first date of attendance was noted as 03/25/19. 2. Documentation of a physical exam was not available for review in the record of child #5, child's record noted first date of attendance was 08/29/18. This was a follow-up review to verify documentation was obtained by the center, this was previously cited on the inspection report of January, 2019.

Plan of Correction: Staff will ensure documentation of a phsical examination is obtained for each child at the time of attendance

Standard #: 22VAC40-185-60-A
Description: Based on a sample review of children's records, the center failed to ensure the following information was complete in each record: name, address and phone number of each custodial parent and if applicable, work phone number and place of employment. Evidence: Documentation reviewed in one of five records (child 2) identified joint legal custody, information was not complete for both individuals.

Plan of Correction: Staff will review records to ensure all required information is complete in each child's record.
