Comments:
An unannounced monitoring inspection was conducted on 5/23/19 from 9:10 am until 11:00 am. At the time of the inspection 14 children were present with two teachers. The sample size consisted of two staff's records and five children's records. Children and staff were observed during free indoor play, snack preparations, bathroom break, hand washing, transitions and behavioral guidance. Violations were found during this inspection and are documented on the violation notice. If you have questions or concerns contact the licensing inspector at (540) 292-5933 for further assistance.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility failed to ensure all hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: The bathroom used by children in the preschool classroom has a high open shelf. A spray bottle of bleach solution and a spray bottle of sanitizing cleaner (H2 Orange) was on the open shelf in the bathroom.

Plan of Correction: The cleaners will be moved to a locked location. All staff will be advised that hazardous substances must be kept in a locked location.

Standard #: 22VAC40-185-540-C
Description: Based on a review of the first aid kit, the facility failed to purchase all the required items for the first aid kit. Evidence: The first aid kit in the preschool classroom only contained one triangular bandage when at least two are required.

Plan of Correction: A triangular bandage will be purchased.

Standard #: 22VAC40-80-120-E-1
Description: Based on observation, the facility failed to ensure the current license is posted in a place conspicuous to the public. Evidence: The license posted in the classroom expired May 2018.

Plan of Correction: The current license will be posted.

Description: Based on observation, the facility failed to ensure the most recent inspection of the facility is posted.

Evidence:

The inspection results posted are dated 4/16/18. The most current inspection prior to today's inspection was dated 12/17/18.
