Technical Assistance:
Technical assistance was provided in the following areas of the standards: 22VAC40-191 (Background checks); 22VAC40-185-(3)-190 (Program Director qualifications); 22VAC40-185-(3)-190 (Program Leader qualifications); 22VAC40-185-(8)-560 (Nutrition); 22VAC40-185-(8)-580 (Transportation)

Comments:
An unannounced monitoring inspection was conducted on 5/22/19 from 2:50pm - 4:15pm. At the time of entrance there were 30 school age children in care with six staff. Children were observed participating in various activities in the gym, doing homework, and eating snack. Records were reviewed for five children while at the facility. The records for five staff were reviewed on 5/23/19 at the central office. Currently there is no medication at the facility. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-550-C
Description: Based on a review of documentation and interview, it was determined that the facility did not ensure that the emergency evacuation and shelter-in-place procedures/maps shall be posted in a location conspicuous to staff and children on each floor of each building. Evidence: 1. The maps for emergency evacuation and shelter-in-place were posted in multiple locations within the facility. However the procedures for emergency evacuation and shelter-in-place were not posted anywhere in the facility. 2. Staff #3 confirmed that the procedures for emergency evacuation and shelter-in-place were not posted anywhere in the facility.

Plan of Correction: The facility responded: The procedures for emergency evacuation and shelter-in-place will be posted.

Standard #: 22VAC40-185-550-M
Description: Based on a review of one injury report and interviews, it was determined that the facility did not ensure that the written record of children's serious and minor injuries includes all of the required items. Evidence: 1. One injury report reviewed did not include the method of how the parent was notified and the time the parent was notified. 2. Staff #3 reviewed the injury report and confirmed it did not include all of the required information.

Plan of Correction: The facility responded: Staff will be re-trained to complete all sections of the injury report form. Center management will review all injury reports to ensure they are complete.
