An unannounced Monitoring inspection was conducted in the center on May 17, 2019. There were 12 children in attendance on the date of inspection. The children's ages ranged from pre-school to school age. Documentation of the fire and health inspections was current. A sample of 4 children's records and 3 staff records was reviewed. The children were observed during free play. The children ate a snack which was brought into the center from home. Findings were reviewed with the Program Lead. Standard #: 22VAC40-185-140-A Description: Based on a sample review of children's records, the center failed to obtain documentation of a physical examination for a child whom has been enrolled in the program over 30 days. Evidence: Documentation of a physical examination was not available for review for child #3, date of enrollment noted as 01/19. Plan of Correction: Staff will notify the child's parent and will obtain documentation of a current physical examination for record retention.
Standard #: 22VAC40-185-40-E Description: Based on a review of children's records, the center failed to ensure center services and activities were maintained in compliance with terms of the current license issued by the department. Evidence: 1. Based on documentation reviewed in one of four records, the center failed to ensure an enrolled child was the appropriate age as indicated on the license for enrollment in the center. The age range permitted by the building official and defined on the license is 2years6months through 12years11months. 2. Child #3's first day of attendance was noted as 1/23/19, the child was 2years5months. Plan of Correction: All information will be reviewed closely prior to an enrollment to ensure the enrollment is within the terms of the license.
Standard #: 22VAC40-185-60-A Description: Based on a sample review of children's records, the center failed to ensure the following information was complete in each child's record: When applicable, work phone number and place of employment of each parent who has custody, 5. Name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached Evidence: The records of children 2,3,4 do not have complete address information for the two emergency contacts; the record of child 4 is not complete with Mother and Father's work phone numbers. Plan of Correction: Staff will request information from each parent to complete the records.
