Parents should list two emergency contacts that are not the parents, even if there are two separate households.
An unannounced monitoring inspection was conducted with center staff on Friday, May 17, 2019 to determine compliance with the licensing standards. There were 4 infants present with one staff, seven toddlers and twos with one staff; and nine preschool children present with two staff. The children were waking from rest, having afternoon snack, coloring pictures, and boarding the center bus to go home. Snack served today: goldfish and juice. Four children's files and one staff file were reviewed - please see Supplemental Summary for details. The facility does not currently administer medication. All staff are current in first aid and CPR training. Evacuation drills have been conducted monthly; the last shelter in place drill was conducted11/30/2018. Standard #: 22VAC40-185-160-C Description: Based on review of one staff file, the facility failed to have documentation of a repeat tuberculosis (TB/PPD) test or screening for each staff. Evidence: the last documented tuberculosis test for Staff A expired 12/21/2018. Plan of Correction: Staff A will get a repeat tuberculosis test or screening.
Standard #: 22VAC40-185-280-E Description: Based on observation, the facility failed to store cleaning materials and insecticides physically separate from each other. Evidence: in Building B, there was a gallon of Insect Killer, a Lysol spray can, and a spray bottle of bleach water solution stored on the same shelf in an upper cabinet outside the bathroom. Plan of Correction: Pesticides and cleaning materials will be stored physically separate from each other.
Standard #: 22VAC40-185-350-C Description: Based on observation, the facility failed to maintain staff to children ratios applicable to the youngest child in the group when children are regularly in ongoing mixed age groups. The ratio for toddlers is 1 staff for every 5 children. Evidence: the toddlers and twos are regularly combined in the same classroom. and the youngest child in the group is 21 months. There were seven children with one staff person from approximately 2:30 p.m. to 3:30 p.m. today. Plan of Correction: At least two staff will be present when there are more than five children present in any grouping that includes a toddler.
