Comments:
An unannounced monitoring inspection was completed on May 10, 2019 from approximately 10:30a.m. to 12:30p.m. Nine children were in care and three staff were on premises during the inspection. Staff and children were observed in the classroom and outside engaged in activities to include lunch and instructional time. Interviews and interactions were conducted and five children?s records and five staff records were reviewed. The Director and an Administrator were available for the inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations:
Standard #: 22VAC40-185-260-A
Description: Based on a review of records and interview, the licensee failed to provide to licensing an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: 1. A fire inspection report dated April 3, 2018 was observed and a current inspection report was not produced. 2. Administrator #2 reported the annual fire inspection has not been scheduled as the inspector wanted to get on a different schedule.

Plan of Correction: The Human Resources Administrator will call the appropriate fire marshal and get the inspection scheduled.

Standard #: 22VAC40-185-550-M
Description: Based on a review of records, the licensee failed to maintain all required elements of a written record of children's serious and minor injuries in which entries are made the day of occurrence. Evidence: 1. A sample of injury reports since the beginning of the school year were reviewed. 2. None of the injury reports contained the date, time and how the parent was notified of the injury. 3. Administrator #1 acknowledged these reports did not contain this information.

Plan of Correction: The Administrative Assistant will add a line to the current form used to capture this information.
