Comments:
An unannounced monitoring inspection was conducted beginning at 1:00 p.m and ending at 3:45 p.m. with the director. There were 49 children ranging in age from 2 months to 5 years of age under the care and supervision of 6 staff. Infants were observed being fed, sleeping and in various infant play equipment. Toddlers, two year olds, three year olds, four year olds and five year olds were observed during nap time. Classrooms and supplies were observed. The playground and play areas were observed. Eight children's records, four staff records including one new staff were reviewed. Eight medications, authorizations and administration logs were reviewed. Emergency drills were up to date. Violations:
Standard #: 22VAC40-185-510-A
Description: Based on review of documentation, the center failed to obtain parental authorization to administer for prescription medication. Evidence: An administration log for Child #3 indicated a ProAir inhaler had been administered on 4/25/19, 4/29/19, 5/6/19, 5/7/19 and 5/13/19. There was no documentation from the parent to administer the medications on these dates. A short term authorization was dated 3/27/2019 and expired after 10 working days.

Plan of Correction: The permission form may have been misplaced in the child's classroom. The medication will be sent home this evening. In the future, if the medication is required permission forms will be on file.
