Technical Assistance:
Technical Assistance was offered regarding obtaining appropriate written permission from parents for the transportation of school age children.

Comments:
An unannounced inspection was conducted in response to a complaint received on 5/15/19. The inspection was held from 3:35pm to 5:15pm. The complaint was related to the transportation of school age children, use of playground, classroom ratios and a child related injury that occurred at the center. Staff members were interviewed, staff and children?s records were reviewed, and classrooms and playgrounds were observed. The center?s vehicles were also observed as school age children were being transported from their respective schools to the center for after school care. Based on the preponderance of evidence, the complaint is deemed valid. Violations not related to the complaint have also been cited. Areas of non-compliance are listed in the Violation Notice. Description: Based on records review, staff did not ensure that the center?s policies and procedures were followed as required by the licensing standards. Evidence: 1. The center?s transportation policy states that ?Each bus must have a list of children riding and a total number in the vehicle?. The policy also states ?Name to face attendance must always be practiced when transporting children?. 2. The center?s transportation policy requires staff to meet the requirements listed under the Pre Certification form if being considered as a bus driver. The Pre-certification list requires a valid driver?s license and states, ?Driver candidates must have a current and valid driver?s license, with the correct endorsements or license class if needed?. 3. On 5/21/19, Staff # 1 was observed arriving at the center with 13 school age children from their respective elementary schools for after school care. Staff # 1 was driving a 24 passenger bus that requires the bus driver to have a commercial driver?s license. a. Staff # 1 did not have a list of children and a total number in the vehicle while the children were being transported. Staff # 1 also did not have the name to face attendance sheets during the transportation. b. Staff # 1 driver?s license indicated she did not have the valid endorsement on her license required to drive a 24 passenger bus.

Plan of Correction: The list of all students being transported are in the bus and will remain in the bus at all times. The bus requiring the CDL was driven due to information received that a CDL bus could be driven without a CDL as long as less than 14 passengers were being transported. The bus is no longer in use.

Description: Based on record review, parental permission was not secured for children being transported. Evidence: 1. On 5/21/19, Child # 1 (5 years old) was transported to the center from his respective elementary school for after school care. 2. The center?s child enrollment form requires parents to complete a section that includes permission to transport and additional transportation information. The section was not completed or signed by Child # 1?s parents. The center did not have written parental permission to transport Child # 1.

Plan of Correction: The form for the child was misplaced and a new form was completed immediately.
