Comments:
A Subsidy Health and Safety inspection was completed on May 29, 2019 from approximately 10:45a.m. to 12:30p.m. to review supplemental health and safety requirements. Thirty-four children were in care and six staff were on premises during the inspection. Staff and children were observed in classrooms and on the playground engaged in activities to include outdoor free play, toileting, hand washing, lunch time and preparation for nap time. Interviews and interactions were conducted and medications were reviewed. Seven children?s records and six staff records were reviewed. The Owner/Director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee.

Description: Based on a review of staff records, the licensee failed to obtain documentation that a staff is free of communicable tuberculosis submitted at the time of employment and prior to coming into contact with children for one of six staff. Evidence: 1. Staff #1's record, hired May 8, 2019, lacked tuberculosis screening/test results. 2. Administrator #1 acknowledged this staff has not obtained a tuberculosis test/screening.

Plan of Correction: The new staff member hired on May 8, 2019 is scheduled to be screened for TB on Tuesday, June 4, 2019.

Description: Based on a review of records, the licensee failed to include a component in the emergency preparedness plan regarding continuity of operations procedures to ensure that essential functions are maintained during an emergency. Evidence: 1. The emergency preparedness plan lacked a component as to how continuity of operations procedures to ensure essential functions are maintain during an emergency will occur. 2. Administrator #1 acknowledged this was not included in the emergency preparedness plan.

Plan of Correction: Procedures for the continuity of operations to ensure that essential functions are maintained during an emergency will be documented and included in our emergency response plan as soon as possible. As director, I will e-mail this documentation to our current licensing inspector for approval.
