An unannounced monitoring inspection was conducted today from 1:45pm-3:35pm. There were 32 children in the direct care and supervision of 5 staff. The first aid kit, emergency materials, evacuation drills, a sample of 4 children's records, a sample of 4 staff records, and the physical plant was reviewed. The children were observed resting during the nap time period, using the rest room and washing hands to have snack (animal crackers and milk). The air conditioning was broken during the time of the inspection, however appropriate cooling mechanisms (fans) were in place to maintain an appropriate temperature in the facility until repairs can be made next week. Standard #: 22VAC40-185-140-A Description: Based on a review of children's records, each child did not have a physical examination by a physician. Evidence: 1. Child #3 (date of enrollment: 1/3/2017) did not have a physical examination on file during the time of the inspection. Plan of Correction: We will ask the parents for a copy of the physical examination.
Standard #: 22VAC40-185-60-A Description: Based on a review of children's records, 1 of 4 children's records was incomplete. Evidence: 1. Child #3 was missing documentation of an annual update. The last one was on 1/3/2017. Plan of Correction: We will have the parents update the emergency information.
Standard #: 22VAC40-185-450-A Description: Based on observation, all children do not have linens consisting of a top and a bottom cover during the designated rest period. Evidence: 1. In the Pre-K room, 2 out of 18 children did not have a top cover (blanket) during the designated rest period. Plan of Correction: We will make sure they all have a top and bottom cover during the rest period.
