Comments:
An unannounced Monitoring Inspection was conducted today from 11:55am to 1:20pm. There were 102 infants to schoolagers directly supervised by 14 staff. The physical plant, 4 staff records, 5 children?s records, children's medications and medication authorization records were current from previous inspection visit, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed taking naps and participating with an outdoor video game vendor. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Standard #: 22VAC40-185-440-E
Description: Based on review, the facility did not ensure that occupied cots, beds and rest mats are at least 12 inches apart. Evidence: Observed 11 cots that were occupied and less than 12 inches apart.

Plan of Correction: Will correct at the end of nap.
