Comments:
An unannounced monitoring inspection was conducted today from 7:30am-9:15am. At the start of the inspection there were 24 children and 28 more arrived throughout the morning for a total of 52 children. The children were observed reading, playing cards games, doing arts and crafts, and playing group games in the gym together. A sample of 4 children's records, 5 staff training records, 2 medications, the first aid kit, emergency materials, evacuation drills, and the physical plant were reviewed. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of children's records, 1 out of 4 children did not have a physical examination by a physician. Evidence: 1. Child #1 did not have documentation of a physical examination on file during the time of the inspection.

Plan of Correction: The child's parents were contacted during the inspection to request a copy of the physical examination.
