Comments:
An unannounced monitoring inspection was conducted on 05/23/2019 from 3:45pm - 4:50pm. At the time of entrance there were 55 school age children in care with six staff. Children were observed during transition from public school into the licensed program. Hand washing was observed prior to afternoon snack which was served in the cafeteria. Large motor games were observed in the gym. Records were reviewed for eight children. Injury documentation and procedures, medication and emergency supplies additionally reviewed. The violations are listed on the violation notice and were discussed with the program director at the conclusion of the inspection. The provider's responses for the "plan of correction" were not received as of 06/04/2019 and will not appear on this Violation Notice.

Standard #: 22VAC40-185-340-A
Description: Based on observation, the center failed to ensure that when staff are supervising children, they shall always ensure their care and protection. Evidence: 1. The inspector observed staff 1, staff 2 and staff 3 supervising children's hand washing at approximately 4:00pm. a. The children in the group were from the kindergarten and first grade age group. b. The hand washing took place in the rest rooms located on the opposite side of the school from the cafeteria. 2. The inspector asked each staff how many children were in the group. a. Staff 1 stated there were between 23 and 25 children in the group. b. Staff 2 stated she did not know how many children were in the group. c. Staff 3 stated there were 25 children in the group. e. Each staff stated they did not have an attendance sheet with them. Staff 1 stated that the attendance sheets were maintained in the cafeteria which is the designated space for the after school program. 3. After children completed hand washing the inspector conducted a count of children and it was determined there were a total of 28 children in the group. 4. The lack of knowledge regarding the number of children in care put children at risk in the event of an emergency or unforeseen event.

Description: Based on observation, the center failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff. Evidence: 1. Staff 1, staff 2 and staff 3 were observed walking a group of 28 children ages, kindergarten - first grade, from the back area of the school to the cafeteria. a. When the children left the rest rooms all three staff were positioned at the front and middle area of the line. 2. This resulted in a group of nine children being out of sight and sound supervision of staff when the line turned a corner for approximately 30 seconds.

Description: Based on observation, the center failed to ensure that when centers choose to provide snacks the following shall apply: Centers shall follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). Evidence: 1. Children were served Motts Assorted Fruit Flavored Snacks for afternoon snack. a. Fruit flavored gummy snacks do not meet USDA guidelines. 2. A second food component was not served to children. a. USDA requires snack to consist of two separate food components.
