Comments:
The licensing inspector conducted a inspection on 05/09/19 in response to a self-report made by the center regarding allegations in the area of supervision. Interviews were conducted with staff , records were reviewed and observations of the program were made relating to the allegation. The information gathered during the investigation supports the allegation. Violations are noted in the violation notice.

Violations:
Standard #: 22VAC40-185-40-E
Description: Based on a record review, interviews and observations, the center did not ensure the center's activities, services, and facilities were maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department. Evidence: 1. The center's bus procedures obtained on 05/09/19 stated, "Staff will take the following steps when unloading children from the bus: The Director, other assigned person or classroom teacher will escort the children to their room." It further states, "Classroom teacher will check them "in" on the classroom roster, using the face-to-name procedure, touching each child." 2. In an interview with the director, it was confirmed, staff are required to physically escort children to their classroom after being unloaded from the bus. The director further confirmed, on 04/23/19 child #1 was not physically escorted from the bus into the classroom and was not "checked in," by the classroom teacher. 2. According to the written incident report dated 04/23/19 and interviews with staff, between 11:00am and 12:00pm, a six year-old child #1 was, "left unattended" and "inproperly transition(sic) student in classroom."

Plan of Correction: The following corrective action was provided by the director during the exit interview: Staff #1's employment was terminated immediately. Staff were retrained on supervision policies in addition to transportation policies for all staff.

Standard #: 22VAC40-185-340-F
Description: Based on a self-reported incident, interviews and observations, the center did not ensure children under 10 years of age were always within actual sight and sound supervision of staff. Evidence: 1. On 04/23/19 between 11:00am and 12:00pm, six year-old child #1 was unloaded from the center's bus during the mid-day bus drop-off by staff #1. 2. Based on an interview, the director verified viewing video footage of child #1 after the mid-day drop-off on 04/23/19. The director confirmed child #1 was unloaded from the bus and entered her classroom. The director further confirmed, there were no staff inside the classroom at that time, resulting in child #1 being without sight and sound supervision for three minutes.

Plan of Correction: The following corrective action was provided by the director during the exit interview: Staff #1's employment was terminated immediately. Staff were retrained on supervision policies in addition to transportation policies for all staff.
