Description: Based on direct observations, the center failed to ensure equipment of the center was maintained in a clean, safe and operable condition. Evidence: An upholstered chair in the school-age classroom was in bad repair. The material on the chair seat was torn, foam padding was exposed on the seat.

Plan of Correction: New cushion Fill purchased and upholstery cover ordered online on 03/12/19, should arrive by Thursday..

Description: Based on direct observations, the center failed to ensure hazardous substances such as cleaning materials were kep in a locked place using a safe locking method that prevents access by children. If a key is used, the key shall not be accessible to the children. Evidence: The Licensing Inspector observed an aerosol can of disinfectant in the school age bathroom The disinfectant was within reach of the children. 2. Center staff was informed by the LI, staff removed the disinfectant from the area upon notice.

Plan of Correction: All chemicals will be kept away from children, under lock and key.

Description: Based on direct observations, staff interview and review of staff records, the Center failed to ensure in each grouping of children at least one staff member who meets the qualifications of a program leader or program director was regularly present. Evidence: The records reviewed for staff in the Toddler's classroom and in the Two-year classroom did not meet support that each individual met qualifications as Program Leader.

Plan of Correction: Staff schedules will be revised to ensure a qualified program leader is assigned each day to each classroom. Additional staff with classroom experience will begin the training module for Program Lead and will be promoted upon completion. Staff who is the Program Lead in that classroom will discontinue working 4-Ten Hour Days and resume the 9am-6pm Monday thru Friday schedule to sustain adequate coverage.

Description: Based on direct observations, the Center failed to ensure procedures for the administration of a nonprescription medication was consistent with the manufacturer's instructions for age, duration and dosage. Evidence: An over-the-counter medication for infant pain and fever for child b. did not have a physician's order to administer, the manufacturer's instructions for under age 2 indicated to consult a physician. The child is under the age of two years.

Plan of Correction: Medication was returned to parent at pick up.

Description: Based on direct observations, the center failed to ensure each first aid kit was easily accessible to staff but not to children. Evidence: The first aid kit was observed hanging in the school-age classroom within reach of the children.

Plan of Correction: Relocate the first aid kits to the kitchen and closet. Items that were listed as "Keep out of Reach" were removed from Playground bags so now they just have gauze, band-aids, etc. like a portable playground bag.

Description: Based on a sample review of staff records, the Center failed to obtain a search of the central registry from the state in which an individual had resided in the preceding five years for any founded complaint of child abuse or neglect against him. Evidence: One of six reviewed records, staff #3, noted an out of state Ohio residence in the past 5 years. Documentation of an Ohio Search of the Central Registry was not available for review. The individual has been employed in the center since January, 2019.

Plan of Correction: Out of state background check not requested by previous Director. We have tried to obtain her CPS from Brown County however Web Check does not list any juvenile offenders online, there is no other online/automated way to check, so we have left a voicemail for them to call us back in regards to the next step.
