An announced monitoring inspection was conducted on June 6, 2019 to review staff records. The licensing inspector reviewed administration, staff qualifications and training, background checks and licensure and registration procedures. The sample size consisted of three staff records. Areas of noncompliance are identified on the Violation Notice.

Standard #: 22VAC40-185-160-C Description: Based on record review, in one of three staff records reviewed, the licensee did not ensure that staff resubmit tuberculosis (TB) test results every two years. Evidence: Program Coordinator verified that the most recent TB results for Staff #2 were dated 6/2/17. Plan of Correction: Staff member will have the results next Tuesday.
