Technical Assistance:
**Discussed requirements for medication authorization forms, transportation policy, including staff hire dates on records and documenting annual review of children's records.

Comments:
An unannounced renewal inspection was conducted today from 10:30am-12:15pm. There were 38 children directly supervised by 9 staff. The physical plant, 5 staff records, 5 children?s records, 5 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group dramatic play, reading books, playing outside, and engaging with staff. There was a supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas of the child day center were maintained in a safe and operable condition. Evidence: 1. On the playground a wooden garden planter had broken exposing splintering wood. 2. The metal shed on the playground has warping panels exposing sharp metal along the bottom edge of the structure.

Plan of Correction: We are aware of the broken plater and are planning to fix it as soon a possible. We will find a way to repair the shed or to ensure the sharp areas of the shed are inaccessible.

Standard #: 22VAC40-185-280-B
Description: Based on observation, not all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: Cleaning materials were observed in an unlocked cabinet in Classroom 1.

Plan of Correction: We will lock the cabinet.

Standard #: 22VAC40-185-510-G
Description: Based on record review, not all children's medication was in the original container with the direction label at ached. Evidence: 1. Child 3 and and Child 5's medications were not in the original container and with a pharmacy label attached.

Plan of Correction: We will ask the parent's to provide the perception label.

Standard #: 63.2(17)-1720.1-B-3
Description: Based upon documentation reviewed, an out of state child abuse and neglect result was not obtained. Evidence: Two of the five staff records reviewed states that Staff 1 and 2 have lived out of the state of Virginia in the preceding 5 years, and no out of state child abuse and neglect results were obtained (Staff 1, DOH: 8/24/2018; Staff 2, DOH: 2/5/2019).

Plan of Correction: We will submit the request for the background check.
