63.2(17) License & Registration Procedures

Comments:
An unannounced mandated Monitoring inspection was conducted today from 10:00 am to 1:45 pm. There were 29 children present with 5 staff. Children were observed reading books in circle time, playing with cars and blocks, and having lunch (pizza, corn and mixed fruit). Reviewed 3 staff files, 3 children's files, medications and authorizations, fire and health inspections, emergency evacuation log, and physical plant. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, children's files are incomplete. Evidence: Child#1 is missing documentation of annual child updates and confirmation of up-to-date information in the child's record.

Plan of Correction: We will have the parent update.

Standard #: 22VAC40-185-210-A
Description: Based on record review, staff did not have documentation of Program Leader qualifications available for review. Evidence: Staff#1 and Staff#3 were missing documentation of Program Leader qualifications.

Plan of Correction: I will get the certificates or transcripts and scan them to you when we receive them.

Standard #: 22VAC40-185-240-A
Description: Based on record review, staff did not receive training by the end of their first day of assuming job responsibilities. Evidence: Staff#1, Staff#2 and Staff#3 did not have documentation of having completed Child Abuse and Neglect training, or Playground Safety Training.

Plan of Correction: We will make sure they have the training

Standard #: 22VAC40-185-240-D-3
Description: Based on record review, staff did not have documentation of satisfactorily completed a training course for rectal medication administration. Evidence: Child#4 has emergency medication required to be administered rectally and no one on site is trained in this procedure.

Plan of Correction: We will contact our nurse to have people trained.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center, inside and outside, are not being maintained in a clean, safe and operable condition. Evidence: 1. Observed light fixtures with exposed glass bulbs missing plastic protective covers in the Preschool 1 and PreK rooms. 2. Observed peeling paint and exposed sheet rock from a ceiling water leak in the office area.

Plan of Correction: The maintenance people are coming on Saturday to fix the ceiling and lights

Standard #: 22VAC40-185-510-A
Description: Based on record review, prescription medication is being administered without current written authorization from the parent. Evidence: Child#1 medication authorization for Pro-Air is expired. (January 2019)

Plan of Correction: We will get a new one from the parent.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review, staff did not have a national child abuse clearance for any other state in which the individual has resided in the preceding five years. Evidence: Staff #3 (date of hire 3/5/19) did not have documentation of a child abuse clearance for Washington D.C.

Plan of Correction: We will send it by end of business Monday
