Comments:
The licensing inspector conducted an unannounced monitoring inspection on Friday, June 7, 2019 to determine the program's compliance with licensing standards. The inspector arrived at 3:30pm and concluded the inspection at approximately 5:30pm. The program's site director was present and assisted the inspector throughout the inspection. The census for today's inspection consisted of 88 children in the direct care of 11 staff. During the inspection, children and staff were observed participating in a variety of activities. The children were observed eating their choice of dinner or snack available to the program. They were later oberved as they separated into different groups in the cafeteria. A group of children were observed as they prepared to go on a filed trip to a nearby roller skating rink. The remainder of the children were observed as they participated in the program's end of the year dance recital. Staff interacted in a positive manner with children and parents. All areas of the facility to include the cafeteria, hallway and bathrooms were inspected. The facility was equipped with age and stage appropriate materials and supplies. All required posted information including the license, last inspection notice, daily schedule, evacuation/ shelter-in-place map & procedures, emergency numbers, and news letter were posted. The first aid kit and non-medical emergency supplies were inspected and found complete. The last documented evacuation drill was held on 06/03/2019 and the most recent shelter-in-place drill was held on 04/17/2019. Eight children's records were reviewed. Additional components of staff records are stored off site and will be reviewed by licensing inspectors at a later date. Standard #: 22VAC40-185-140-A
Description: Based on a review of eight children's records and interview on 06/07/2019, the center did not ensure one child obtained documentation of a physical examination by or under the direction of a physician prior to attendance or within one month after attendance. Evidence: 1) The record for Child #2 (DOE: 08/15/2017) did not contain documentation of a physical examination. 2) During interview, the director confirmed the physical examination for Child #2 could not be located.

Plan of Correction: Per the Center: Will try to locate the missing physical and will also request a recent copy from the parent. In the future, the documentation will be required at the time of enrollment.

Standard #: 22VAC40-185-60-A
Description: Based on a review of eight children's records on 06/07/2019, the center did not ensure that five children's records contained the required information. Evidence: 1) The record for Child #1 (DOE: 09/05/2017) did not contain annual documentation of parent confirmation that the required information was up to date. The information was last updated on 09/05/2017. 2) The record for Child #3 (DOE: 08/15/2017) did not contain a complete address for the two emergency contacts listed. 3) The record for Child #4 (DOE: 09/12/2015) did not contain a complete address for the two emergency contacts listed. Each child record shall contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. 4) The record for Child #6 was missing documentation of viewing proof of the child's identity and age, a complete address for the two emergency contacts listed, and the child's first date of attendance. 5) The record for Child #7 was missing documentation of the child's first date of attendance.

Plan of Correction: Per the Center: The missing information will be requested form the parents. A system will be put in place to ensure first dates of attendance are accurately documented and addresses include the required information.
